Premier Wellness Initiative (PWI) is a wellness and women’s health practice offering services that support individuals through pregnancy, postpartum, weight management, and overall wellness. These Terms govern all in-person and virtual services, including consultations, classes, treatments, and online resources.
2. Acceptance of Terms
By scheduling an appointment, completing a registration form, or making a payment, you acknowledge that you have read, understood, and agree to these Terms and Conditions. If you do not agree, please do not use our services.
3. Services and Appointments
All services require a confirmed appointment and completed intake forms prior to treatment.
Appointment availability may vary depending on provider schedules and service type.
PWI reserves the right to reschedule or cancel appointments due to provider emergencies, weather, or unforeseen circumstances.
Clients are required to arrive on time. Late arrivals of more than 10 minutes may result in rescheduling or a shortened session.
4. Payments and Fees
Payment is due at the time of booking or service, unless otherwise arranged.
PWI accepts major credit cards, debit cards, and approved payment links.
Pricing for services and programs is subject to change without notice.
For classes, your spot is secured once payment is received.
5. Cancellations and Refunds
We require at least 24 hours’ notice for cancellations or rescheduling.
No-shows or late cancellations may result in a non-refundable fee or forfeiture of deposit.
Refunds are only provided for cancellations initiated by PWI or under exceptional circumstances approved by management.
Digital downloads, classes, and educational sessions are non-refundable once accessed.
6. Health and Safety
Clients must disclose relevant medical conditions, medications, or contraindications before any service.
PWI providers reserve the right to refuse or modify services based on safety concerns or professional judgment.
Our facility follows all applicable health, sanitation, and privacy standards.
7. Telehealth and Virtual Services
For virtual consultations, clients must ensure a private and secure environment.
PWI is not responsible for connectivity issues or interruptions beyond our control.
Telehealth services are conducted using HIPAA-compliant platforms to protect your confidentiality.
8. Intellectual Property
All content provided by PWI — including educational materials, class recordings, written guides, and website content — is the property of PWI and protected by copyright laws.
You may not copy, distribute, or reproduce any materials without prior written permission.
9. Limitation of Liability
While we strive to provide accurate information and quality care, PWI and its affiliates are not liable for:
Any indirect, incidental, or consequential damages arising from the use of our services or materials
Outcomes based on incomplete or inaccurate information provided by clients
Technical issues or service interruptions beyond our control
10. Privacy and Confidentiality
All client information is handled in accordance with our Privacy and Confidentiality Policy. We uphold strict HIPAA-compliant standards to ensure your data remains protected.
11. Modifications to Terms
PWI reserves the right to modify these Terms at any time. Updates will be effective immediately upon posting to our website. Continued use of our services constitutes acceptance of any revised Terms.
